DAY CARE EXAMINATIONSEach child attending a day care center shall be examined by a licensed physician prior to the date of admission or within thirty days after the date of admission. If the child is less than six years old, he or she shall also be examined by a licensed physician annually thereafter from the date of admission. Pre-admission examinations must be conducted in the six months prior to the date of admission if the child is younger than three years old. For children three years or older, pre-admission examinations must be conducted in the twelve months prior to the date of admission. OAC § 5101:2-12-58 The examining physician shall affirm that the child has had the immunizations required by Ohio law for admission to school or has had the immunizations required by the State Department of Health for infants and toddlers. In addition, the examining physician shall affirm one of the following:
A physician shall be required to provide written instructions for the administration of medication or food supplements by the day care center for any child in attendance. Written instructions are not required for the following: prescription medication (if properly labeled); non-prescription fever-reducing medications that do not contain aspirin; non-prescription cough or cold medications that do not contain codeine; and nonprescription topical ointments, creams or lotions.
