If the child is less than six years old, he or she shall also be examined by a licensed physician annually thereafter from the date of admission. Pre-admission examinations must be conducted in the six months prior to the date of admission if the child is younger than three years old. For children three years or older, pre-admission examinations must be conducted in the twelve months prior to the date of admission. OAC § 5101:2-12-58

  3. That based upon an examination that meets the requirements of the United States Department of Health and Human Services for participation in that department's Head Start Program, the child is in a suitable condition for enrollment in a child day care center. OAC § 5101:2-12-58
