Application for Licensure Examination, Limited BranchesUpon request by any interested individual, the State Medial Board of Ohio shall furnish an application for examination and certification in the appropriate limited branch. OAC § 4731-1-09 (A) All applicants for licensure in a limited branch of medicine or surgery shall file a written application under oath on the forms prescribed by the State Medical Board of Ohio, and provide such other facts and materials as the Board requires. OAC § 4731-1-09 (B) No application shall be considered filed until the appropriate fee has been received by the State Medical Board of Ohio. OAC § 4731-1-09 (C) All application materials submitted to the State Medical Board of Ohio by applicants for licensure or certification will be thoroughly investigated. The Board will contact individuals, agencies or organizations for recommendations or other information about applicants as the Board deems necessary. Applicants may be requested to appear before the State Medical Board of Ohio or a representative thereof as part of the application process. OAC § 4731-1-09 (D)
