Assumed Name, Practice Under

No person holding a certificate to practice any limited branch or branches of medicine or surgery may conduct a practice under any name or title, either as an individual, company or concern, except under his own name or that mentioned in the certificate. OAC § 4731-1-03 (G)

“The State Medical Board of Ohio…shall, to the extent permitted by law, limit, revoke or suspend a certificate, refuse to register or refuse to reinstate an applicant, or reprimand or place on probation the holder of a certificate for one or more of the following reasons: 

  1. Permitting one’s name of one’s certificate of registration to be used by a person, group or corporation when the individual concerned is not actually directing the treatment given.” ORC § 4731.22  (B)(1)
